Metalurg managed to hold off Shkendija for a second place finish as both clubs lost in this round, which featured many goals to close the 2011/12 season.
Vardar, with a goal in the third minute of stoppage time, won the derby against Metalurg to close the season with another win. Vardar was presented the league trophy and rewarded its fans with a good finish to conclude a successful season. Metalurg initially took the lead just before half time when defender Zlatko Tanevski scored an own goal, but Vardar came back after the break with one goal apiece by Filip Ivanovski and Igor Stojaković. Despite the loss, Metalurg kept its one point advantage and finished in second place as Shkendija was defeated 0:3 by Napredok in Kichevo.
Renova destroyed relegated side Ohrid 1:7 on the thanks to a hat-trick by Boban Janchevski and a brace by both Muharem Bajrami and Ardžent Gafuri. Meanwhile, Horizont Turnovo got past Rabotnichki 5:4 in a game that went back and forth and saw a lot of goals. Striker Saša Popin scored a hat-trick for the winning side, while Borche Manevski scored a brace for Rabotnicki. Another hat-trick occurred in Shtip by attacker Lazar Iliev who netted three goals to lead Bregalnica to a 4:1 win over 11 Oktomvri. In the remaining game of the round, Teteks squeaked past Sileks 2:1 to enter the promotion/relegation play-off on a high note.
