Studeniçan by drawing lots, Hotla, Llopati 97 and Besa (S) after winning in the play-offs qualified for the next season of third league group North.

Five fourth league group winners entered the draw for four available spots. The bottom three teams got relegated from the North, but one more place got opened as champions Shkëndija 77 won promotion through the play-offs. It was decided that one lucky team will escape the play-off by drawing lots and win a direct promotion. That lucky team was Studeniçan, the winner of Kisela Voda group.

In the Round 1 matches Hotla defeated Besa (S) on penalties, while Llopati 97 celebrates over Bujkovci. Than the two losing sides clashed for the last place with Besa (S) this time winning on penalties over Bujkovci.
