Unlike the other groups, Prilep features a play-off for determining the champions. After the conclusion of the round robin format, the top four enter the champion play-off. This season the first-placed plays against the third, while the runner-up against the fourth-placed team.

The bottom placed team plays against the runner up of the 5.MFL Prilep. The winner will secure a place in the 4.MFL Prilep for the next 2026/27 season. The fifth league in the group was won Bratstvo 07 from Zhitoshe, who won promotion.
