Goblen against Saraj and Sopishte against Vlaznimi Orizari will feature in the play-offs to fill the remaining two spots of the Third League North for next season.

Five fourth league groups battled for promotion to the Third League North.  The five winners this season were:

OFS Skopje – Saraj
OFS Kisela Voda – Sopishte
OFS Gazi Baba – Trubarevo
OFS Kumanovo – Goblen
OFS Lipkovo – Vlaznimi Orizari

In the draw, Trubarevo was the luckiest team as they avoided the play-offs altogether and went directly into the Third League, while the remaining four teams will participate in a single game play-off with the two winners gaining promotion.  The two matchups are the following:

Goblen vs. Saraj
Sopishte vs. Vlaznimi Orizari

Both games will be played on 17 June at the Boris Trajkovski stadium in Skopje.
